 JENSEN BEACH, FL – Dr. Rafael Olazagasti, 65, passed away peacefully at his home in Jensen Beach, Florida on March 4, 2012.Dr. Olazagasti was born December 25, 1946 in Santruce, Puerto Rico and graduated from the University of Puerto Rico Medical School. He completed his residency in Family Practice at Madegan Army Medical Center, Fort Lewis, Washington and was honorably discharged as Major in 1977.He practiced Family Medicine in Latham, NY from 1977 to 1991 and was Part-time Assistant Professor at Albany Medical College. Following completion of his MBA, Dr. O (as he was referred to by many) served as Physician Hospital Surveyor for the NYS Department of Health. He was on various committees within the state including the position of Chairman of the New York State Board of Medicine and New York State Board of Medical License Restoration/Moral Character Panel and New York State Patient Occurrence & Tracking System (NYPORTS). He was also on the Advisory Board of Welch-Allyn. Dr. O was Medical Director of Saint Mary’s Hospital, Amsterdam, N.Y, North Jefferson Health Systems in Watertown, N.Y., Vice President of Medical Affairs with the Samaritan Medical System, Watertown, N.Y., and also at Benedictine Hospital, Kingston, N.Y. He was President of Olazagasti Healthcare Consulting Services, LLC and provided consultative and management services to healthcare and educational institutions in the U.S. and abroad. Dr. O was Consultant Dean for Academic Affairs at the American University of Antigua School of Medicine.
Throughout his vast career, Dr. O found great reward in mentoring and supporting family members, friends, and others in their pursuit of medical education. He was always available to offer advice and gave freely of his time and energy. He is fondly remembered for his wonderful outgoing personality and decisive nature.
He semi-retired to Florida in 2010 and began enjoying his lifelong passion for boating. Golf was a new sport which he was determined to master and enjoyed many hours at the range and on the course. His hardy laugh and zest for life will be sorely missed.
Dr. Olazagasti was predeceased by his first wife, Patricia Wilsey and his parents, Adelaida and Rafael Olazagasti. Survivors include his loving wife, Christine of Jensen Beach, his devoted son, Rafael and Tonya of Baltimore, Maryland, his step sons, George and Paul Winters, his brother and closest friend, Roberto and his wife Ileana of Albany, NY, his grand children whom he adored, Rafael, Bella, and Lily Anne , his nieces Aileen and her husband Steven Nicole, Drs. Aixa and her husband Allen Alvaraz, and his nephew, Roberto Olazagasti and Lew Breckenridge and his great nieces and nephews. Our family has a void that will never be replaced.
Funeral services will be held 9 AM Saturday, March 10th at St. Patrick’s Church, Ravena. Burial will follow in St. Patrick’s Cemetery, Coeymans. Friends may call at the Babcock Funeral Home, Ravena on Friday from 3 to 8 PM. In lieu of flowers contributions to the chariety of your choice would be appreciated.
